I am about to start an extension and have everything agreed by BC, would now like to add ufh in part of the build. Reading around the problems of screed failure when place directly onto insulation I am looking at putting the insulation under the concrete slab (125mm) as agreed with BC and then place the ufh in the screed (75mm). Has anyone done this and are there any problems/issues. I know that the warm up time will be longer but the heat retention will be greater due to the thermal mass of the concrete slab.

You’ve answered your own question. The build-up you describe is a common one, and will indeed take longer to heat up and cool down. It is a solid approach though.
